High dose, 1000 mcg methylated folate, as the all natural 6S isomer

L-5-MTHF was designed for patients with demonstrated increased need for folate such as those with either the A1298C or C677T MTHFR mutation. It provides 1000 mcg per capsule of folate in the bioidentical, bioactive form, 5-MethylTetraHydroFolate. L-5-MTHF (the "L" signifying that it is the all-natural [6S] isomer) is the active circulating form of folate found in the body and one of the several forms found in food. In nature it is found only as this [6S] isomer, which is why DFH provides 5-MTHF as this natural isomer rather than the mixed (or racemic) form.

L-5-MTHF should be used when high doses of folate are needed.

Individuals who have tested homozygous with either the A1298C or C677T MTHFR mutation, or those with elevated homocysteine levels, should be using L-5-MTHF as the preferred form of folate supplementation. This will be more effective for them than other forms of folic acid or folates.

Others who may benefit from L-5-MTHF:

  • pregnant women and women wishing to become pregnant who have familial history of depression, high homocysteine levels, or genetic defect in MTHFR. Note: All pregnant women should takePrenatal Pro™ as their comprehensive multivitamin; can combine with L-5-MTHF if higher levels of folate are needed.
  • women with abnormal pap smears (precancerous)
  • those with very high homocysteine levels not responding quickly to Homocysteine Supreme™(consider using them together)

  • organic acid testing that reveals elevated FIGLU (marker used to identify folate deficiency)
  • patients with family history of dementia/vascular dementia
  • patients with depression
  • long-term alcoholism
  • * long-term use of oral contraceptives, and persistent use of medications to lower folate levels, including high doses of NSAIDs such as aspirin or ibuprofen; anticonvulsants (phenytoin, phenobarbital, and primidone); trimethoprim (antibiotic), pyrimethamine (antimalarial), triamterene (blood pressure med.) and sulfasalazine (treatment for ulcerative colitis).

    • Hi ~ 4 questions RE folate:

      1. What is the difference between -
      * Metafolin L-5-methyltetrahydrofolate
      * folate as [6S]-5-methyltetrahydrofolate

      2. Does the type of MTHFR one has determine which one works better?
      (I am compound heterozygous C677T, A1298C.

      3. When beginning, what is the best dose to start (1mg/1000mcg daily - for how long till increasing?)

      4. Can capsules be opened and half used at a time? (cost is critical so I can continue taking whatever is needed.)

    • Hello - 1. Metafolin is L-5-MTHF L-5-methyltetrahydrofolate = 6S-5-methyltetrahydrofolate = L-5-MTHF 5-MTHF can mean L-5-MTHF and/or D-5-MTHF. It is not recommended to use methylfolate that does not specify D or L or 6S 2. You should use a methylfolate that uses the L isomer or the 6S isomer (same thing). 3. Always start low and then work up. Also always start with methylcobalamin because without vitamin b12, the methylfolate does you no good. 4. I recommend you not even use capsules to start. I recommend you start with 1/2 tablet of Active B12 with Methylfolate once a day and allow to dissolve under your tongue.       My teenager is on 1 Homocystex Plus and one
      Metanx a day for two mutations of C. Does this
      sound like to high of a dose of metafolate?
      What is your recommendation for someone with an A and C mutation?
      Would L-5 MTHF be sufficient? Would I need to take it with
      P5P and B12 to have it work best?

    • Hello - If your teen is handing the dosages well with HomocysteX Plus and Metanx, then she is good. If she is experiencing side effects (things she wasn't experiencing that are negative once she started taking them), then she is taking too much and needs to reduce the amount. Everyone is different in how much one needs in terms of methylfolate. Someone with an A1298C MTHFR mutation will likely need something like Optimal Multivitamin as this provides comprehensive amounts of various nutrients needed to support the A1298C mutation - including methylfolate and methylcobalamin, b6, b2 and antioxidants. Both should also consider taking: - Optimal Fish Oil: 2 capsules daily with food - ProBiota 12: 1 capsule after dinner - Vitamin D360: 2 drops anytime of day Should you need further help, I'm here! ;) In health, Dr Ben

    • I have just been told that I have 2 copies of the c677t mutation. I was given L-5MTHF to take. I found your website and see that your doctors are very knowledgeable in this area. I am not sure what other blood test should be done, the doctors have not done any other test for my homocysteine levels, I have been to a hematologist, naturopathic and primary and none of them have checked anything else. Can you please advice on what other test should be ordered and what should I be taking. I have just started taking SAM-E and read it lowers homocysteine levels. I look forward to hearing from you, thank you.

      After you read through this article, I would like to emphasize – start slowly and ease into it.  It is very important to understand this point.  If people try to reverse a lifetime of symptoms by swallowing a ton of supplements in high amounts, it is definitely going to backfire.  One at a time and evaluate.  Then increase the amount taken if needed.

    • I have been studying this since 2008 and have read so much information that I am drowning in it.
      I am desperate to find someone who can help me create a nutritional protocol for someone with the A and C defects.
      I have current labs.
      I have been working on gut health for 4 years.
      I have out of control pitting edema and need help.
      I can travel to meet in person in a doctor's office - I am in the Greensboro, NC area but can fly anywhere for help.

      I’m so sorry to hear about your troubles.  In all your research, I’m sure you’ve come across the website Dr. Ben created about MTHFR:  www.mthfr.net  Check it out if you haven’t been there.  Also, to find a practitioner to help you:  www.mthfrsupport.com  There’s a practitioner listed in NC, but it might be good to shop around and check out various practitioner’s websites to locate one that has experience with your specific mutation.
